Summary Description of the IPAC Lapse
The clinic identified deficiencies in the reprocessing of dental equipment. The dental equipment were cleaned with ultrasonic cleaner, packaged and place in the autoclave. However, the autoclave cycle was not initiated. The items were later removed from the autoclave and the instruments were returned to circulation without being properly sterilized.

-Education was provided on proper reprocessing techniques.
-Education was provided on verifying chemical indicators following reprocessing, as well as the appropriate documentation of autoclave parameters and chemical indicator results.
-Education was provided on verifying chemical indicators prior to opening instrument packaging for patient use.
- Review of the clinic’s reprocessing policies and procedures.
