COVID-19 Vaccine Standby List

Please complete this form if you would like to register to be on standby list in the event that we have extra doses of the COVID-19 vaccine at a clinic. 

To avoid vaccine wastage by ensuring as many individuals as needed are available to receive remaining doses at the end of a clinic, the Eastern Ontario Health Unit (EOHU) has extended the eligibility criteria for the standby list to individuals aged 18 or older who have not yet been able to book an appointment to receive the COVID-19 vaccine.

COVID-19 vaccination clinics throughout the EOHU region are operating efficiently and ethically to ensure that vaccine wastage is minimized. All vaccine doses at each site are planned for and allocated. Should doses remain at the end of a clinic due to missed or cancelled appointments, individuals on the standby list could be called in to receive the extra doses to avoid vaccine wastage at local clinics.

Who is eligible to register for the standby list

Standby registration for a COVID-19 vaccine is open to individuals who will be 18 or over in 2021 (born in 2003 or earlier) who:

  • have not yet received their first dose;
  • have not yet booked an appointment; and
  • can get to a vaccine clinic within 20 minutes of receiving a call.

When you register, you must enter a valid phone number. This is how you will be contacted if there are extra doses of the COVID-19 vaccine at one of our vaccination clinics. 

Completing your standby registration

Standby registration can only be done online. A proxy (e.g. spouse, support worker, friend) can complete the registration for you. 

NOTE: Individuals who are part of a provincially-defined eligible group should attempt to book their COVID-19 vaccine appointments through the provincial booking system or another vaccination partner (e.g. pharmacy, participating primary care provider, or other). The standby registration list is not a booking tool. Appointments will only be offered if there are unused doses available at the end of a clinic. 

Please note that your registration information may be shared with vaccination partners or the province for scheduling purposes.

To complete your standby registration, the following information is required: 

  • your contact information 
  • your Ontario health card number
  • a home or mobile phone number where you (or your proxy) can be reached
    • NOTE: You must be able to get to the clinic within 20 minutes of receiving a phone call

Review and submit your information

Make sure your information is accurate, especially your phone number. Your phone number is how you will be contacted if there are extra doses available. 

Complete your registration

Once you have reviewed your information for accuracy, click on the ‘Submit’ button to complete your registration. Your registration has been successfully completed when a confirmation message appears on your screen. If you provided a valid email address, you should receive an email to confirm your registration. Please DO NOT CALL the EOHU to confirm your registration. We appreciate your patience.
